Job summary

VitalCaring Group is seeking a Weekend PRN Home Health Registered Nurse. You will lead patient care from admission to discharge, delivering skilled nursing in patients' homes across a designated territory.

This autonomous role emphasizes strong clinical judgment, education, and seamless coordination with care teams. You will manage plans of care, collaborate with physicians, and reduce hospitalizations while supporting recovery and long-term independence.

Qualifications

• Active RN license and CPR certification required.
• Graduate from an accredited nursing program.
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficient with EMR systems and documentation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including meds, treatments, and interventions.
• Serve as case manager coordinating across therapists, aides, and physicians.
• Monitor progress and make clinical decisions based on changes in condition.
• Educate patients and families for disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate EMR documentation.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
