Richwood Nursing & Rehab in Oldham County, Kentucky is seeking a PRN Speech Language Therapist to evaluate and treat residents across the facility. You will document services and coordinate with Families, Physicians and the health team to ensure high quality care.

Responsibilities include developing treatment plans, supervising CFY/SLP assistants when applicable, discharge planning, and participating in care conferences.

Qualifications

• Bachelor's degree in Speech Language Pathology is required; graduates of higher degrees preferred.
• Proficient documentation and data entry in clinical software.
• Licensed Speech Language Therapist with state licensure

Responsabilités

• Evaluate patients within 24 hours of physician referral.
• Develop and obtain approval for individualized treatment plans.
• Supervise CFY/Speech Language Pathologist Assistants in direct care.
• Communicate progress with families, physicians and health team members.
• Record evaluations, daily notes, and progress updates per policy.
• Discharge planning and caregiver training as needed
