Job summary

Eastern Health Group in the United States seeks a PRN Speech Language Pathologist (SLP) to assess and treat patients requiring rehabilitation services in our skilled nursing and rehabilitation facilities.

You will establish and administer a treatment program with goals tailored to each patient, document progress, and educate patients and families in therapy methods. Collaboration with the care team is essential to deliver high-quality, patient-centered care.

Qualifications

• Graduated from an accredited Speech Therapy program.
• Valid professional license and registration.
• Experience in long-term care and geriatric settings preferred.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to determine condition and rehab potential.
• Establish and administer treatment plans with goals according to patient capacity.
• Set realistic, achievable goals and document patient progress.
• Educate patients and families in therapy methods.
• Complete evaluations, progress notes, recertifications and discharges.
• Collaborate with disciplines to provide patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.
