Creative Solutions in Healthcare, Inc. seeks a Per Diem Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care.

The SLP will evaluate swallowing and cognitive deficits, implement treatment plans, and provide AAC solutions while coordinating care with other professionals. Responsibilities include education for residents and families, timely documentation and billing, and ensuring compliance with regulatory requirements.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsabilités

• Evaluate residents' swallowing or cognitive deficits.
• Help residents achieve cognitive or swallowing goals.
• Education for residents and caregivers.
• Provide skilled speech-language therapy assessments and develop treatment plans.
• Develop and implement individualized treatment plans to increase independence.
• Provide AAC systems for severe expressive/language disorders.
• Provide training to clients, families, caregivers, and other professionals.
• Coordinate diagnostic swallowing procedures as ordered by physician.
• Demonstrate competence in Speech Therapy standards of practice.
• Communicate changes in residents' condition.
• Provide accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other staff.
• Ensure resident safety and report any abuse or neglect.
• Observe confidentiality of patient information.
• Comply with legal and professional requirements.
