Job summary

Elara Caring is seeking a Speech-Language Pathologist to join our home-based care team. You will perform initial and ongoing evaluations, develop plans of care, and deliver direct speech-language therapy in patients’ homes. Travel within the branch coverage area is required.

The role emphasizes collaboration with physicians and care teams to ensure high-quality, patient-centered care. A valid SLP license and a relevant Master’s degree are required, with opportunities to advance and access

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.

Responsibilities

• Promote Elara Caring’s mission and policies to ensure quality of care.
• Perform initial therapy evaluations and re-evaluations per OASIS and PPS guidelines.
• Provide direct speech-language pathology services in home care settings.
• Develop and adjust the Plan of Care with physician orders and patient goals.
• Communicate findings and changes to the care team and supervisor.
• Maintain accurate therapy notes and logs and ensure proper discharge planning.
