Job summary

Norwichtown Rehab and Healthcare in Norwich, CT is seeking a Speech-Language Pathologist (SLP) on a PRN basis. You will evaluate and treat residents with communication, cognitive-communication, and swallowing needs in a collaborative rehabilitation setting.

Responsibilities include developing individualized plans, tracking progress, educating families, and coordinating care with the interdisciplinary team. A CT license and Master's in SLP are required, with SNF/post-acute experience preferred.

Qualifications

• CT license required
• Master's degree in Speech-Language Pathology from an accredited program
• Strong clinical, communication, and organizational skills
• Skilled nursing or post-acute rehabilitation experience preferred

Responsibilities

• Complete evaluations for speech, language, cognitive-communication, and swallowing disorders
• Develop individualized treatment plans based on resident needs and clinical findings
• Provide skilled interventions addressing communication, cognition, and dysphagia
• Track resident progress and modify treatment approaches as needed
• Educate residents, families, and caregivers on communication and safe swallowing strategies
• Maintain clear and timely clinical documentation
• Coordinate care with nursing, therapy staff, and other healthcare professionals
