Job summary

Skyview Rehab and Nursing in Wallingford, CT is seeking a Speech-Language Pathologist (SLP) for a PRN opportunity to help residents strengthen communication, cognitive, and swallowing abilities.

The successful candidate will evaluate residents, develop individualized treatment plans, provide skilled therapy, monitor progress, educate families, and document accurately, collaborating with the interdisciplinary team in a supportive rehab environment.

Qualifications

• Active Connecticut Speech-Language Pathologist license required.
• Master's degree in Speech-Language Pathology from an accredited program.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Evaluate residents for speech, language, cognitive-communication, and swallowing needs.
• Develop individualized treatment plans based on clinical findings and resident goals.
• Provide skilled therapy addressing communication, cognition, and dysphagia.
• Monitor resident progress and adjust interventions based on response to treatment.
• Educate residents, families, and caregivers on communication and safe swallowing strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, rehabilitation staff, and other members of the interdisciplinary team.
