Job summary

Visalia Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. The role focuses on evaluating and treating residents with communication, cognitive, voice, and swallowing disorders to help them reach their highest level of independence and quality of life.

The SLP will collaborate with nursing, dietary, therapy, physicians, and other team members while maintaining thorough evaluations, progress notes, and discharge summaries in

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current California SLP license in good standing.
• CCC from ASHA preferred; CPR/BLS certification.
• Experience in skilled nursing or long-term care preferred.
• Strong communication, clinical, and documentation skills.

Responsibilities

• Evaluate residents’ speech, language, cognition, voice, and swallowing abilities.
• Develop and implement individualized treatment plans.
• Provide therapy for communication, cognitive, and swallowing disorders.
• Complete and document swallowing evaluations.
• Monitor residents’ progress and modify treatment plans as needed.
• Educate residents, families, caregivers, and staff on communication and swallowing strategies.
• Collaborate with nursing, dietary, therapy, physicians, and other interdisciplinary team members.
• Participate in care planning and discharge planning as needed.
