Job summary

Oasis Therapy Services in Crest Hill, IL, is seeking a Speech-Language Pathologist (SLP) to provide one-on-one rehabilitation at Willow Falls in Crest Hill and Eden Vista Rock Run in Joliet, IL.

The PRN position supports residents with communication, cognitive, voice, and swallowing needs, using Net Health EMR and traveling between sites. We offer flexible scheduling, competitive compensation, and opportunities for growth.

Qualifications

• Active IL Speech-Language Pathologist license.
• Master's or doctoral degree in Speech-Language Pathology.
• CCC-SLP preferred.
• Current CPR certification.
• Geriatric, dysphagia, skilled nursing, or rehabilitation experience preferred.

Responsibilities

• Evaluate and treat communication, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans with measurable goals.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on communication strategies and swallowing precautions.
• Complete accurate Net Health documentation and collaborate with the care team.

• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, dysphagia, skilled nursing, or rehabilitation experience is preferred.
• Reliable transportation and ability to travel between Crest Hill and Joliet.
