Oasis Therapy Services in Illinois seeks a PRN Speech-Language Pathologist to provide rehabilitation services at one or more locations, including Willow Falls and Eden Vista Rock Run. The role focuses on evaluations, treatment planning, and delivering skilled therapy to residents in an ALF setting.

The ideal candidate will hold an IL SLP license, be CCC-SLP preferred, and demonstrate strong communication and teamwork. CPR, valid driver's license, and ability to work independently are required.

Qualifikationen

• Active and current SLP license in Illinois.
• Master's or doctoral degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification preferred.
• CPR certification current.
• Valid Minnesota driver's license.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and with residents, families, and healthcare professionals.
• Experience in geriatrics, assisted living, skilled nursing, or rehabilitation preferred but not required.

Aufgaben

• Perform comprehensive speech-language pathology evaluations using appropriate assessment methods.
• Develop individualized treatment plans with measurable short- and long-term goals.
• Assess and treat communication, cognitive, voice, and swallowing disorders.
• Provide skilled therapy interventions to improve residents' functional abilities and quality of life.
• Monitor residents' progress and modify treatment plans as needed.
• Communicate residents' progress, recommendations, and concerns to families, caregivers, and the interdisciplinary care team.
• Educate residents, families, and caregivers on communication strategies, swallowing precautions, and follow-through programs.
• Complete clinical documentation and charge entries accurately and on time.
• Assist with resident scheduling as needed.
• Maintain therapy equipment and work areas in a safe and clean condition.
• Follow all facility policies, safety procedures, and professional standards.

Kenntnisse
