Job summary

Baylor Scott & White Health in Dallas, TX is seeking a Social Worker MSW 1 to educate and advise patients and families on coping with emotional and medical needs, coordinating crisis intervention, bereavement, referrals, and discharge planning.

You will develop and implement psychosocial assessments and collaborate with the care team to improve patient outcomes, meeting Texas social work standards and maintaining professional documentation.

Qualifications

• Master's degree in Social Work.
• Texas licensure: LCSW, LMSW, or LMSW-AP.
• At least 1 year of experience in social work.
• TX licensure required to practice.

Responsibilities

• Conducts psychosocial assessments and documents findings.
• Develops and evaluates treatment plans with education for patients/families.
• Provides care plans including crisis intervention, referrals, and discharge planning.
• Liaises with community resources and healthcare team for outcomes.
• Prepares professional reports and case summaries.
