Job summary

Qualifications

• Eligibility for TX licensure must be maintained.

Responsibilities

• Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of patients.
• Develops, implements, and evaluates treatment plans; provides education to patients and families.
• Prepares care plans including resource coordination, crisis intervention, prevention, education, patient advocacy, bereavement support.
• Collaborates with healthcare team to transition and place patients within the service line.
• Participates in performance improvement initiatives and chart review.
