Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a Social Worker for PRN and as-needed shifts. You will interview and assess patients and families, coordinate internal and external services, participate in case reviews, and collaborate with the care team to ensure adherence to treatment plans.

The role requires a Bachelor's in Social Work with a preference for a Master's, KY licensure (CSW/LPCA/LCSW) within 6 months, and Handle With Care certification within 30 days.

Qualifications

• Bachelor's degree in social work is required.
• Master's degree preferred.
• Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire.
• Certifications: Handle With Care within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

Education

Bachelor's degree in Social Work

Master's degree preferred
