Job summary

Lifepoint Health in Towanda, PA is seeking a PRN Social Worker to assess and plan interventions that help patients cope with social, emotional, economic, and environmental challenges, while supporting discharge planning and coordinated care.

You will interview patients and families, coordinate internal and external services, and provide therapy, case management, and team-based care across the treatment continuum.

Qualifications

• Bachelor's degree required; master's preferred.
• Licensure: Board Certified Social Worker or related field within 6 months of hire.
• Handle with Care certification within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or family, caregivers, and legal representatives.
• Determines and arranges internal/external services and interventions.
• Participates in case reviews to evaluate progress and compliance with the treatment plan.
• Assists with discharge planning and identifies resources for transport, housing, healthcare, and social/spiritual needs.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.
