Job summary

Lifepoint Health is seeking a Social Worker for PRN shifts to support patients and families with comprehensive social services, discharge planning, and therapy within our care teams.

The role emphasizes collaboration with clinicians, case reviews, and community resource referrals to promote patient well-being and safer transitions of care.

Qualifications

• Bachelor's degree required; Master's degree preferred.
• Board Certified Social Worker or related licensure.
• Handle With Care certification within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or family, caregivers, or legal representatives.
• Determines and arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate progress and consults with healthcare team to ensure treatment plan compliance.
• Assists with discharge planning and identifies resources (transportation, housing, healthcare, social/spiritual services).
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

How you’ll contribute:

Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Essential Functions

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

Qualifications and requirements:

• Education: Bachelors required; Masters preferred
• Licenses: Board Certified Social Worker, or related field (i.e. counseling)
• Certifications: Handle With Care within 30 days of hire.

Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
