Job summary

IU Health Arnett Hospital is seeking a PRN Social Worker to provide face‑to‑face psychosocial assessments, crisis intervention, discharge planning, and resource coordination for patients and families. You will serve as a resource for students and staff, collaborating with a multidisciplinary team to create comprehensive treatment plans addressing psychosocial needs.

The role includes liaison work with protective services, and on‑call/holiday rotations may be required.

Qualifications

• Master’s Degree in Social Work (MSW) required.
• Permanent licensure as an LCSW in Indiana required.
• Certification in Crisis Prevention Intervention or equivalent de‑escalation training within 6 months of hire.
• Knowledge of evidence‑based psychosocial assessment, short‑term counseling, and crisis intervention.

Responsibilities

• Meet face-to-face with patients and families for psychosocial assessments.
• Assist in development of educational materials for team members, patients, and families.
• Serve as resource and role model for students and staff.
• Collaborate with the multidisciplinary team to develop and implement treatment plans.
• Interpret and communicate complex patient and family needs, modeling caring practices.
• Assess psychosocial risk factors, prior functioning, and support systems.
• Counsel patients and families regarding emotional, social, and financial impacts of illness.
• Access and mobilize community resources to meet patient needs.
• Provide mental health assessments upon physician referral.
• Facilitate complex discharge planning to ensure safe transitions.
• Serve as liaison for protective services in child/adult/elder abuse cases.
• Maintain crisis intervention and de‑escalation practices for acute crises.
• Provide intervention in guardianship, foster care, adoption, and mental health placements.
• Offer specialized mental health services, including crisis intervention and psychiatric referrals.
• Support patients in end‑of‑life, guardianship, and adoption cases.
• Use interviewing principles with victims, perpetrators, families, and significant others.
• Develop relationships with hospital admins, medical teams, law enforcement, APS/DCS, schools, and community agencies.
• Document all clinical activities accurately in EMR.
