Summary/Objective

Concerto is seeking a PRN Social Worker to join its team! The PRN Social Worker provides psychosocial evaluation and counseling to patients residing in Skilled Nursing Facilities or rehabilitation centers regarding dialysis-related issues. The role involves collaboration with the multidisciplinary treatment team to incorporate psychosocial needs into care planning, documentation in accordance with facility policies, and facilitation between the dialysis unit and the SNF Social Services Department. The position reports directly to the Manager of Social Services and functions as a member of the Interdisciplinary team.

Essential Functions

1. Conduct psychosocial assessments on each patient and develop treatment plans aligned with medical treatment plans per CMS guidelines (initial, 90-day, annual, and as needed for unstable patients). Track CIPA/POC for SNFs and coordinate POC meetings with RN, RD, MD, and the patient.
2. Complete KDQOL and other required assessment tools.
3. Provide monthly documentation for each patient in the caseload.
4. Educate new patients about their Rights and Responsibilities, Grievance Procedures, and document completion.
5. Track patients for QAPI monthly.
6. Maintain professional relationships with staff at Concerto and the skilled nursing facilities. Participate in administrative and staff programs, including meetings and in-service training.
7. Develop educational materials for patient and staff education as needed.
8. Handle special projects as assigned.

Qualifications

1. Master's degree in social work.
2. Healthcare or dialysis-related background preferred.
3. Licensed as an LSW or LCSW.
4. Valid driver's license.
5. Intermediate computer skills; proficiency with MS Word, Excel, Outlook, and Concerto-specific applications within 60 days of employment.

Physical Demands

1. Ability to lift at least 20 lbs.
2. Attention to mental, visual, and auditory cues regarding patients, staff, and environment.
3. Work environment typical of healthcare facilities, with temperature control and moderate noise.
4. Potential exposure to infectious materials.
5. Travel between facilities required.
