Minimum Education And Experience Required
- License/Registration/Certification: IL-LSW Eligible, LCSW preferred issued by the State of Illinois/WI-Social Work Certification issued by the State of Wisconsin
- Level of Education: Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
- Years of Experience: 1 year of social work experience in hospital social work setting.
Major Responsibilities
- Provides social work services to patients, families, and individuals including psychosocial assessment, screening, needs evaluation, appropriate interventions and follow-up, and discharge planning. Implements interventions based on assessment, tailored to patient needs and resources.
- Participates in multidisciplinary team efforts to coordinate care, including post-acute placement, palliative/hospice services, medical equipment, home healthcare, outpatient follow-up, mental health resources, and community services. Advocates for patient involvement and coordinates interventions with team members.
- Stays informed about community resources, legislation, and regulations affecting healthcare delivery. Educates patients and families on these issues, provides resources for discharge and post-acute care, and connects patients with services for bereavement, ethical issues, directives, and end-of-life concerns. Collaborates with agencies on social matters and ensures access to community services.
- Communicates effectively with patients, families, healthcare team, and community agencies. Documents activities per protocols and standards.
- Manages patient stays to optimize length of stay and appropriateness of care level. Coordinates care across the continuum to reduce readmissions. Participates in staff orientation and education.
- Educates patients and families on Advance Directives, assists with documentation, and participates in legal proceedings as needed.
- Maintains professional standards per accreditation, hospital, system, state, and NASW standards. Completes ongoing education to maintain licensure and expertise.

Physical Requirements And Working Conditions
- Travel required; exposure to weather and road conditions.
- Operates necessary equipment.
- Works in office and patient care environments, with potential exposure to hazards and infectious diseases, requiring PPE adherence.
- Weekend, holiday, and evening shifts as required.
- Sitting, standing, speaking, reading/writing, and walking throughout the day.
- Fast-paced environment with time constraints.
