Responsibilities

• Assist patients and families with health-related financial, social, and emotional concerns, and help develop effective coping skills.
• Provide social work services according to the plan of care.
• Ensure timely communication of changes in the patient's clinical status to the healthcare team and care center staff.
• Educate patients, families, and team members on medical social work interventions and treatment plans.
• Perform care planning, transfer, and discharge functions.
• Identify risks or threats to the patient's health in their environment and implement appropriate interventions to reduce risks, communicating findings to the physician and team.
• Other duties as assigned.

Qualifications

• Master's or doctorate degree in social work from an accredited school.
• At least one year of social work experience in healthcare.
• Meets state and federal requirements.
• Current CPR certification.
