Job summary

Grove Park Healthcare Center in New Jersey is seeking a PRN Speech Language Pathologist (SLP) to join our team. The SLP will improve residents' communication and swallowing abilities and work within a collaborative rehab team.

Responsibilities include assessments, developing care plans, data collection, documentation in EMRs, and educating residents and families. Remote monitoring and interdisciplinary collaboration are part of the role.

Qualifications

• State licensure as a Speech-Language Pathologist.
• CPR/BLS certification.
• Proficiency in Section GG data collection and documentation requirements, as applicable.
• Strong communication and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Modality/Vital Stim certification +

Responsibilities

• Assess and evaluate residents' speech, language, cognitive, and swallowing abilities.
• Develop individualized intervention plans to address communication and swallowing difficulties.
• Ensure accurate Section GG data collection for applicable assessments.
• Administer speech therapy interventions, including exercises and swallowing therapy.
• Maintain timely documentation of resident progress and interventions (EMRs).
• Educate residents, families, and care staff on safe mobility techniques and RTM with discharged patients.
• Collaborate with rehab staff (PT/OT) for holistic care.
