Grove Park Healthcare Center is seeking a PRN Speech-Language Pathologist (SLP) to improve residents' speech, language, cognitive, and swallowing abilities at our New Jersey facility.

You will perform assessments, develop individualized plans, collect Section GG data, deliver therapy, and document progress in EMRs. Collaboration with Physical and Occupational Therapists supports holistic care, with remote check-ins for continuity.

Qualifications

• State licensure as a Speech-Language Pathologist.
• CPR/BLS certification.
• Proficiency in Section GG data collection and documentation requirements, as applicable.
• Strong communication and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Modality/Vital Stim certification +

Responsabilités

• Assess and evaluate residents' speech, language, cognitive, and swallowing abilities.
• Develop individualized intervention plans for communication and swallowing difficulties.
• Ensure accurate collection of Section GG data for assessments.
• Administer speech therapy interventions and swallowing therapy.
• Maintain documentation of resident progress and interventions in EMRs (Net Health/PCC).
• Educate residents, families, and staff on mobility techniques and RTM check-ins.
• Collaborate with rehab staff to provide holistic care.
