Discovery at Home in Naples, FL is seeking a PRN Physical Therapist to perform comprehensive home health assessments, establish treatment plans with physician input, and deliver physical therapy using exercise, modalities, and patient education.

The role requires graduation from an approved PT program, active Florida license, CPR, one year of home health experience, and reliable transportation. Background screening applies; EOE.

Formación

• Graduate of a Physical Therapy program approved by the AMA/APTA.
• Licensed or registered in the applicable state without restrictions.
• One year of home health experience required.

Responsabilidades

• Provides initial and ongoing comprehensive assessments of the client’s needs, including OASIS assessments.
• Establishes a treatment plan in consultation with the physician and revises it as necessary.
• Treats clients to relieve pain, restore function, and maximize performance using appropriate PT modalities.
• Teaches, supervises, and counsels family and client in the physical therapy program.
• Instructs client in use of mobility devices and assistive devices.
• Supervises PT assistants and paraprofessionals at least monthly.
• Participates in case conferences, staff meetings, and in-service programs.
