Job summary

JourneyCare Hospice is seeking a Hospice RN to join our team in Barrington, IL. New competitive salaries and immediate openings are available.

QUALIFICATIONS: Hospice experience required; RN licensed in the state; CPR certified; valid driver’s license. Night shifts 7PM-7:30AM with differential and mileage reimbursement offered.

Qualifications

• Maintains a current CPR certification.
• Ability to deal tactfully with patients, family, visitors, agency personnel and the public.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assess patient and family caseload, including admission processing and care planning.
• Obtain data and assess information from patient factors and coordinate with interdisciplinary team.
• Coordinate care by communicating with the attending physician and other agencies.
• Provide visits to reassess patient needs and update the plan of care.
• Perform nursing procedures as directed and assist with death pronouncement and family support.
• Maintain accurate patient records and timely care plans.

HOURS

At Least 2 Shifts/Month (Preferably more) 7PM-7:30AM

LOCATION

Barrington, IL

Now offering up to $46.63/hour

Night Shift Differentials: Additional $2.50/hour during the week nights and additional $5/hour on a weekend night

What You’ll Do:

• Assist with the responsibility for patient and family caseload, including the assessment, determine eligibility, admission processing, planning, implementation and evaluation phases of the nursing process


• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team


• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care


• Provide visits to re-assess the patient's needs and update the plan of care


• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician


• Perform clinical pronouncement of death and provide support to family


• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
