Job summary

• Strong assessment and clinical skills
• Excellent communication and interpersonal abilities
• Ability to travel throughout Metro Atlanta

Responsibilities

• Conduct comprehensive 60-90 day in-home client assessments
• Evaluate patient health status, needs and care plans
• Communicate assessment findings to the care team
• Provide skilled nursing care as needed: medication management, wound care, catheter and tube feeding care, diabetes management and related nursing duties
