ProHealth Home Health & Hospice in Birmingham, AL seeks a PRN (RN) Home Health Case Manager to deliver skilled nursing services in patient homes as per physician orders and plan of care. You will conduct initial evaluations, coordinate care, and update plans while ensuring quality and compliance.

The role requires an active RN license and ability to exercise independent judgment. You will work part-time, travel to patient homes, and participate in on-call coverage as needed.

Competenze

• Graduate of an approved school of professional nursing and currently licensed.
• Two years’ nursing experience preferred.
• Ability to exercise initiative and independent judgment.
• Ability to accept agency philosophy and goals.

Mansioni

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and reevaluates nursing needs.
• Initiates plan of care and revisions as needed.
• Provides services requiring specialized nursing skills.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each visit.
• Coordinates services and communicates changes to the physician.
• Counsels patient and family in meeting nursing needs.
• Participates in in-service programs and adheres to agency policies.
• Processes orders, notifies physician of changes, and handles certifications.
• Determines amount and type of nursing care required.
• Refer to therapies for patients needing specialized skills.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences and participates in QA/PI as assigned.
• Provides total patient care as needed and covers on-call duties.
• Completes and submits OASIS assessments and discharge summaries.
• Uses ICD-10 codes appropriately.
