ProHealth Group is seeking a PRN RN Home Health Case Manager in Hoover, AL to deliver intermittent skilled nursing care in patients' residences under physician orders and supervision. The role involves assessing needs, initiating and updating the care plan, and coordinating with the care team within the Home Health & Hospice setting.

The position requires an active RN license and CPR certification, with preferred two years of nursing experience.

Formación

• Graduate of an approved nursing program and licensed in applicable state(s).
• Two years nursing experience preferred.
• Ability to exercise initiative and independent judgment.

Responsabilidades

• Provide skilled nursing services per plan of care.
• Perform initial evaluation visits and reevaluations.
• Initiate and revise the plan of care as needed.
• Provide services requiring specialized nursing skills and rehabilitative procedures.
• Document visits and care summaries promptly.
