Job summary

Responsibilities

• Provides services in accordance with the plan of care.
• Performs initial evaluation visits and reevaluates nursing needs.
• Initiates and revises the plan of care as required.
• Provides skilled nursing services in accordance with professional standards.
• Maintains timely progress notes and care summaries.
• On-call duties nights, weekends and holidays as assigned.
