Addus HomeCare is seeking a Registered Nurse to join their team in Albuquerque, NM. As a key member of our health care team, you will be responsible for assessing patient needs, developing care plans, and providing direct care to improve patient outcomes.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year of clinical practical nursing experience, preferably in intensive care or medical surgical nursing.
• Home health experience preferred.
• Must have valid CPR certification.

Responsibilities

• Complete initial assessments to determine patient care needs.
• Develop and implement a care plan based on nursing diagnosis.
• Provide direct patient care and communicate with health team members.
• Counsel patients and families regarding care.
• Act as Case Manager when assigned.
• Prepare clinical notes and update providers.
