Job summary

Addus Homecare, Inc. is seeking a Registered Nurse to join our Albuquerque, NM team. This full-time role features a Monday through Friday schedule from 8am to 5pm, with visits and care coordination across the patient’s home setting.

You will assess, plan, implement and evaluate nursing care as part of an interdisciplinary team. Qualified candidates will have an active NM RN license, CPR certification, and at least 1 year of clinical nursing experience; home health experience is preferred.

Qualifications

• Graduate from an accredited RN program and licensed in the state (NM).
• At least 1 year clinical experience, preferably in relevant settings.
• Home health experience preferred.
• Must have CPR certification.
• Must have reliable transportation, valid driver's license and auto insurance.

Responsibilities

• Provide visits as assigned, including assessment, eligibility, admissions processing, planning, implementation and evaluation.
• Coordinate care with interdisciplinary team and communicating with physicians.
• Initiate and update the plan of care based on patient needs.
• Perform nursing procedures, including preventative and rehabilitative care.
• Maintain accurate patient records and timely care plans.
