Job summary

Addus Homecare is hiring a Registered Nurse in Albuquerque, NM to join our growing home health team. You will conduct nursing visits, assess patient needs, and coordinate with the interdisciplinary team to ensure high-quality care.

The role emphasizes care planning, documentation, and collaboration with physicians, with a focus on patient-centered outcomes and family support. Valid NM RN license and CPR are required, plus reliable transportation.

Qualifications

• Must be a licensed RN in NM.
• At least 1 year clinical nursing experience, preferably home health or inpatient.
• CPR certification required.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Provide visits as assigned by Clinical Services Manager, including assessment, eligibility, admissions processing, planning, implementation and evaluation of the nursing process.
• Gather data from patient and family factors to coordinate care with the interdisciplinary team.
• Communicate with attending physician and other agencies to coordinate optimal care.
• Assess patient needs and update the plan of care during visits.
• Perform therapeutic, preventive and rehabilitative nursing procedures as directed by physician.
• Handle clinical pronouncement of death and provide family support as applicable.
• Maintain accurate patient records, charts, progress notes, and care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
