Qualifications

• Bachelor's degree in social work or RN.
• Minimum 2 years social work or case management experience in an inpatient setting.
• Current RN or Social Work licensure or healthcare professional licensure.
• Certification in Case Management or Rehabilitation Nursing preferred.
• Strong organizational and communication skills.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental performance improvement initiatives and data reporting.
• Consults with other staff to coordinate patient care and discharge planning.
• Completes IPoC per CMS guidelines and required documentation.
• Schedules family conferences and communicates progress to patients and caregivers.
• Coordinates weekly care conferences to refine treatment plans.
