Job summary

Hospice of South Texas in Victoria, TX is seeking a compassionate PRN Registered Nurse (RN) to join our clinical team for homecare. The RN will provide skilled hospice care in patients' homes, coordinating with the interdisciplinary team and supporting families through the hospice journey.

Requirements include graduation from an accredited nursing program, Texas RN license, valid driver’s license, CPR, and at least two years of clinical RN experience with hospice exposure.

Qualifications

• Hospice experience required.

Responsibilities

• Perform comprehensive patient assessments and update care plans.

• Educate patients and families regarding care needs and disease progression.
• Maintain accurate and timely clinical documentation.
• Promote compliance with hospice regulations and payer requirements.
• Participate in on-call rotation as needed.

Position Summary

The Registered Nurse is responsible for providing skilled nursing care to hospice patients in their homes or place of residence. This role coordinates patient care with the interdisciplinary team, physicians, families, and referral sources to ensure quality, compassionate care throughout the hospice journey.

Essential Responsibilities

• Perform comprehensive patient assessments to identify physical, emotional, psychosocial, and environmental needs
• Develop, implement, and update individualized plans of care
• Provide direct nursing care and symptom management
• Coordinate services with physicians, aides, LVNs, social workers, chaplains, and other team members
• Educate patients and families regarding care needs and disease progression
• Ensure accurate and timely clinical documentation
• Maintain compliance with hospice regulations and payer requirements
• Participate in on-call rotation as assigned
• Promote HOST values, standards, and quality patient care
• Other duties as assigned

Education

Qualifications

Graduate of an accredited Diploma, Associate, or Bachelor School of Nursing

Licensure

• Current Texas RN license
• Current Texas Driver's License

Experience

• Minimum two years RN experience in a clinical setting preferred
• Hospice experience required
