Hospice of South Texas seeks an experienced Registered Nurse to administer skilled nursing care to terminally ill patients in their homes. The role involves after-hours triage, evaluating patient suitability for Hospice services, and coordinating admissions.

The position requires an RN license, relevant experience, and strong interpersonal and palliation skills to support patients and families in a compassionate care setting.

Qualifications

• Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure and a current State RN license.
• Experience: Two years experience as a Registered Nurse in a clinical setting; hospice or home health preferred.
• Skills: Nursing skills per standards of practice; strong interpersonal skills; palliation/end-of-life care skills preferred.

Responsibilities

• Evaluate and assess patients for Hospice services.
• Carry out admissions for terminally ill patients.
• Respond to patient care visits to assess and treat as needed.
• Triage incoming patient calls after business hours.
• Conduct routine Homecare visits at patients' residence; perform pronouncement visits at death when needed.
• Complete chart audits and provide RN assistance at the Hospice inpatient center.

Job Qualifications

• Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure; Current State license as a Registered Nurse; current State Driver's License.
• Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred.
• Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life.
• Transportation: Reliable transportation. Valid and current auto liability insurance.

Environmental And Working Conditions

Works in patients home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally.

Physical And Mental Effort

Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.

• Conduct pronouncement visits at the time of the death of the patient
• Complete chart audits, as requested
• Provide RN assistance at the Hospice of South Texas Inpatient Center, as needed
