Job summary

Discovery at Home, part of the Discovery Senior Living family, seeks a licensed Home Health Registered Nurse to provide comprehensive nursing care in patients' homes, adhering to professional standards and procedures.

You will assess patient needs, develop and implement individualized care plans, administer medications, monitor progress, and coordinate with a multidisciplinary team while maintaining accurate records and pursuing ongoing education.

Qualifications

• Must be licensed or registered by the applicable state without restrictions.
• BSN or ADN from an accredited nursing program.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver's license and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide comprehensive nursing care to patients per standards and procedures.

Responsibilities:

• Provide comprehensive nursing care to patients, adhering to professional nursing standards, policies, and procedures.

• Administer medications, treatments, and therapies as prescribed by healthcare providers.


• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.


• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.


• Educate patients and their families about treatment plans, medications, and self-care techniques.


• Respond to emergency situations and perform life‑saving interventions promptly and accurately.


• Maintain accurate and up‑to‑date medical records of patients' health history, treatment plans, and progress.


• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.


• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.

Qualifications:

• Must be licensed or registered by the applicable state without restrictions.


• Bachelor's degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.


• One year of home health experience required


• Must have knowledge of Medicare and Medicaid guidelines

• Must have a valid driver's license and reliable transportation.


• Must have a current CPR certification.


• Excellent verbal and written communication skills.


• Basic computer skill proficiency. Ability to use Microsoft Windows.
