Job summary

Discovery At Home LLC is looking for a qualified nurse in Grand Prairie, Texas. This full-time position involves providing comprehensive care, assessing patient needs, administering medications, and educating patients and families. Candidates must possess a valid nursing license and have at least one year of home health experience. Responsibilities

• Provide comprehensive nursing care to patients, adhering to professional nursing standards, policies, and procedures.
• Assess patient needs, develop, and implement individualized care plans to ensure optimal patient outcomes.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.
• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.
• Educate patients and their families about treatment plans, medications, and self‑care techniques.
• Respond to emergency situations and perform life‑saving interventions promptly and accurately.
• Maintain accurate and up‑to‑date medical records of patients' health history, treatment plans, and progress.
• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.
• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.

• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines.

• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.
