Job summary

Discovery at Home is seeking a skilled Physical Therapist to evaluate and design personalized home health and outpatient therapy plans in Texas. You will collaborate with a care team, monitor progress, and educate patients and caregivers to improve mobility and prevent injuries.

Requirements include a Doctor of Physical Therapy, state licensure, CPR, valid driver's license, 1 year home health experience, and proficiency with patient documentation and Medicare/Medicaid guidelines.

Qualifications

• A degree in Physical Therapy from an accredited program.
• Must be licensed or registered by the applicable state without restrictions.
• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a valid driver's license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans.
• Develop and implement appropriate treatment plans based on patient needs and goals.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care.
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes.
