Job summary

Discovery At Home in Houston, TX, seeks a dedicated Registered Nurse to provide comprehensive home health care within assigned IL/AL communities. The role emphasizes patient-centered care, medication administration, and care coordination with an interdisciplinary team.

You will educate patients and families, respond to emergencies, and keep detailed medical records while adhering to infection control and quality standards. The position requires a valid license, CPR, and reliable transportation.

Qualifications

• Must be licensed or registered by the state without restrictions.
• Bachelor’s degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited program.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency (Microsoft Windows).

Responsibilities

• Provide comprehensive nursing care to patients, adhering to professional standards and procedures.
• Assess patient needs and develop individualized care plans.
• Administer medications, treatments, and therapies as prescribed.
• Monitor and document patient progress including vital signs.
• Collaborate with interdisciplinary team to coordinate care.
• Educate patients and families about treatment plans and self-care.

• Maintain accurate and up‑to‑date medical records of patients' health history, treatment plans, and progress.
• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.
• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.
• Build positive working relationships with community leadership, wellness teams, residents, and families.
• Serve as a visible clinical representative of Discovery At Home within assigned IL/AL communities.
• Educate residents, families, and community staff about available home health and supportive services when appropriate.
• Identify potential service opportunities and communicate them to the appropriate Discovery At Home intake or business‑development team.
• Participate occasionally in community events, health fairs, educational programs, or other outreach activities.
• Support community relationship development while maintaining the RN's primary focus on quality patient care.
