Job summary

Evolve Therapy Services seeks a dedicated Physical Therapy Assistant for our in-house Skilled Nursing Facility in Oconto, WI. As PRN staff, you will work under the supervision of a Physical Therapist to deliver resident-focused care and help achieve plan-of-care goals.

You will screen residents, assist with evaluations and treatments, train residents and families, and document progress in NetHealth EMR. Ongoing licensure upkeep and professional development are expected.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules in conjunction with the Physical Therapist and other team members.
• Train resident, family and appropriate staff in the use or application of such equipment.
