Step Up Rehab is partnering with Northbank Center for Rehabilitation and Healing and Lakeside Center for Rehabilitation and Healing in Jacksonville, FL to provide skilled Physical Therapist Assistant services in SNFs. This PRN role supports residents’ functional goals in dedicated therapy gyms, with a focus on safe, patient-centered care.

Requires PTA program completion and Florida license (active or eligible). SNF or post-acute experience is preferred; Net Health knowledge is a plus.

Qualifications

• Graduate of an accredited PTA program.
• Active Florida PTA license (or license-eligible) required.
• SNF or post-acute experience preferred.
• Net Health documentation knowledge preferred.

Responsibilities

• Provide skilled PTA-related therapy services to residents in a Skilled Nursing Facility while collaborating with the interdisciplinary team to improve safety, function, and quality of life.
