Job summary

Step Up Rehab, the in-house therapy provider at Northbank Center for Rehabilitation and Healing in Jacksonville, FL and Lakeside Center for Rehabilitation and Healing, is seeking a Physical Therapist Assistant (PTA) for a PRN role in a Skilled Nursing Facility (SNF).

You will provide skilled PTA services to residents, collaborate with an interdisciplinary team, and help improve safety and quality of life in a modern rehab environment.

Qualifications

• Graduate of an accredited PTA program.
• Active Florida PTA license (or license-eligible) required.
• SNF or post-acute experience preferred.
• Net Health documentation knowledge preferred.

Responsibilities

• Provide skilled PTA-related therapy services to residents in a Skilled Nursing Facility.
• Collaborate with the interdisciplinary team to improve safety, function, and quality of life.
