Job summary

Overland Park Post Acute is seeking a licensed Physical Therapist to lead therapy services in a long-term care setting. The role requires timely patient evaluations within 24 hours of referral, and the development and approval of treatment plans with physician collaboration.

You will supervise PT assistants and staff, document medical records, and participate in care conferences. The position includes discharge planning, caregiver training, and potential DME ordering to support patient

Qualifications

• Bachelor’s degree in Physical Therapy and state licensure required.
• Proficient with Casamba and Point Click Care.
• Ability to read technical procedures and policy manuals.
• Must meet continuing education requirements and stay licensed.

Responsibilities

• Evaluate patients within 24 hours of referral or 1 business day.
• Develop and obtain approval for a treatment plan from the referring physician.
• Treat patients per physician plan and supervise PT assistants and students.
• Record evaluations, daily notes, and weekly progress notes per state act and policies.
• Instruct families and nursing staff in maintenance and caregiver training for discharge.
• Participate in discharge planning and attend care conferences and trainings.
• Facilitate ordering of durable medical equipment as needed and report equipment issues.
