Job summary

Summit Care LLC in Kansas is seeking a Physical Therapist to provide therapy services to patients, families, and facilities under PT guidelines. The role includes evaluation, treatment planning, supervision of assistants, and participation in case management to ensure quality care.

The ideal candidate holds an accredited PT program and a valid state license, demonstrates strong communication, leadership, and problem-solving skills, and adheres to all regulatory requirements and safety policies

Qualifications

• Accredited Physical Therapy Program is required.
• Valid Physical Therapy license to practice in the appropriate state(s).
• Effective written and verbal communication skills.
• Effective leadership and customer relations skills.
• Ability to exercise mature judgment with initiative in analyzing and solving problems.

Responsibilities

• Executes doctor''s orders.
• Provides PT screening and evaluation of patients to determine status.
• Develops and implements treatment plans with measurable outcomes.
• Oversees and maintains therapy records.
• Ensures availability of supplies and equipment.
• Maintains patient schedules as per guidelines.
• Documents to maintain quality of medical records and billing accuracy.
• Participates in patient, family, and facility meetings as appropriate.
• Adheres to emergency procedures, infection control, and safety policies.
• Coordinates/trains facility staff, residents, and rehab staff.
• Initiates new program development focusing on functional outcomes.
• Participates in continuous quality improvement.
• Lifts patients using biomechanical techniques; can lift 50 pounds.
• Able to walk, bend, stand, and reach during shifts.
