TheraCare Home Health is seeking a licensed Physical Therapist Assistant (PTA) for a PRN in-home home health role serving San Antonio and surrounding areas. The PTA will provide skilled interventions under a supervising PT and implement individualized care plans with autonomy in a home setting.

The position emphasizes flexibility and one-on-one patient care, with opportunities to contribute to patient mobility, safety, and independence in the patients’ homes.

Qualifikationen

• Associate degree from an accredited Physical Therapist Assistant (PTA) program.
• Active Physical Therapist Assistant (PTA) license in the state of Texas.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience (preferred).
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Aufgaben

• Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT).
• Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT.
• Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions.
• Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities.
• Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments.
• Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies.
• Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist.
• Complete accurate and timely electronic documentation in compliance with Medicare home health regulations.
• Follow infection control standards and home health safety protocols.
• Participate in QAPI and agency performance improvement initiatives.
