Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to mobility, endurance, pain, and other medical conditions. Based on this, the therapist develops and implements treatment plans, performs interventions to improve patient well-being, and monitors progress.

Key duties include:

• Evaluating patient functional status and therapy needs
• Assisting physicians in developing care plans
• Monitoring and reporting patient responses and condition changes
• Providing treatments to relieve pain and restore or maintain function
• Educating patients, families, and healthcare team members about care plans and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in Tennessee
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
