Job summary

Oasis Therapy Services in Beaver Dam, Wisconsin seeks a Physical Therapist (PT) to deliver one-on-one rehabilitation in an assisted living and memory care setting. PRN schedule with flexible hours and potential for additional assignments.

You will evaluate mobility, design individualized treatment plans, educate residents and caregivers, and document care in Net Health. Wisconsin license, CPR, and geriatric or skilled-nursing experience are preferred.

Qualifications

• Active and current Wisconsin Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.

Responsibilities

• Evaluate mobility, strength, balance, gait, and fall risk.
• Develop and provide individualized treatment plans to improve safe mobility and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices.
• Complete accurate Net Health documentation and collaborate with the care team.
