Oasis Therapy Services in Beaver Dam, Wisconsin is seeking a Physical Therapist Assistant (PTA) to deliver one-on-one rehabilitation in an assisted living and memory care setting.

You will work under the supervision of a licensed Physical Therapist, following established treatment plans to address mobility, strength, balance, and transfers, while educating residents and caregivers and documenting care in Net Health.

Qualifications

• Active and current Wisconsin PTA license.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Provide treatments under the direction and supervision of a licensed Physical Therapist.
• Follow treatment plans addressing mobility, strength, balance, endurance, and transfers.
• Monitor residents' responses and report changes to the supervising Physical Therapist.
• Educate residents and caregivers on exercises, safe mobility, and fall prevention.
• Complete accurate Net Health documentation and collaborate with the care team.
