Qualifications

• Participate in care conferences and update the plan of care.
• Provide patient education and advocate for patient rights.

Responsibilities

• Complete initial assessments and documents as required.
• Assess data for admission and discharge needs, including safety and falls risk.
• Act as patient advocate, obtain signatures and ensure privacy and consent.
• Initiate and update treatment plans and document changes.
• Administer medications safely and document responses and side effects.
• Educate patients about medications and comply with policies.
