Job summary

Premise Health Inc. is seeking a PRN Physician to provide primary care to client employees and their dependents ages 2 and older at our Health Centers in Northern Kentucky.

Hours are as needed, Monday through Friday, with responsibility for episodic, preventive, and acute care, plus referrals and care coordination. The role requires a medical degree, board certification, and active state licenses with DEA, CPR hands-on certification, and at least three years post-residency experience in primary,

Qualifications

• Medical degree and board certification required.
• Active state medical license(s) and valid DEA registration.
• CPR certification with hands-on evaluation required.
• Minimum three years of post-residency clinical experience.
• Experience in primary/urgent/emergency care settings is preferred.

Responsibilities

• Provide episodic, preventive, and acute care to patients.
• Conduct histories and physical exams; prescribe treatments per best practices.
• Develop treatment plans and educate patients on medical issues.
• Provide internal/external referrals when needed.
• Maintain medical records; document care in EMR where used.
• Assist in program development and health center goals.
