Job summary

Emerald Nursing & Rehab Mercy in Omaha, NE seeks a Physical Therapist (PRN) to assess, plan, and implement rehabilitation programs for residents in a skilled nursing facility.

The PT will collaborate with physicians, nurses, and a multidisciplinary team to promote functional independence, pain relief, and safe home or facility-based recovery. Licensure required; prior SNF experience preferred but not required. On-site role with flexible PRN shifts.

Qualifications

• Doctoral or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in skilled nursing or long-term care preferred but not required.

Responsibilities

• Evaluate patients’ physical conditions, mobility, and functional abilities to develop treatment plans.
• Create and implement personalized therapy programs to improve mobility and function.
• Administer therapeutic exercises, manual therapy, and modalities to support recovery.
• Regularly assess and document patient progress and adjust plans as needed.
• Educate patients and families on exercises, safety, and home care strategies.
• Collaborate with physicians, nurses, and other professionals to ensure coordinated care.
• Maintain accurate documentation of evaluations, plans, progress, and discharge summaries.
• Adhere to safety and infection control protocols and facility policies.
