Job summary

Emerald Nursing & Rehab Cozad is seeking a Physical Therapist (PRN) to provide skilled PT services to residents in a long-term care setting. You will assess mobility, develop individualized plans, implement therapy interventions, and document progress for residents and care teams.

Qualifications

• Nebraska licensed Physical Therapist (PT) required.
• Registration with Nebraska Department of Health and Human Services required.
• Strong clinical skills and knowledge of PT principles and practices.

Responsibilities

• Evaluating and assessing residents' physical therapy needs.
• Developing and implementing individualized treatment plans.
• Providing physical therapy interventions, including exercises, activities, and modalities.
• Documenting progress and communicating with residents, families, and healthcare team members.
